Nursing-led care at home for medical conditions

We understand that complex medical care at home requires equipment and other adaptations. Our dedicated regional management team will liaise with doctors, district nurses, occupational therapists, physiotherapists and other health and social care professionals, to manage all health care needs and ensure that a holistic and multi-disciplinary care plan is in place.

Some of the conditions listed below will require the involvement of our Specialist Service team. They will work with our care managers using a multidisciplinary approach to ensure that our carers receive the correct training and support to carry out these interventions.

Many of these tasks are known as delegated healthcare interventions, as they are prescribed and monitored by our NHS colleagues and have delegated the intervention to our carers to carry out on a day to day basis.

Your care manager will discuss with you in further detail at assessment and ensure a detailed plan is in place to support with these interventions if required.

 

Support with medical care can include:

  • Urethral and Supra pubic catheter care
    Our Professional Carers can support you with the flushing of a catheter, to remove any debris that may be in the bladder, which can lead to blocking the catheter, preventing it from draining
  • Stoma – ileostomy care, colostomy care, urostomy care
    Our Professional Carers can support you with the flushing of a catheter, to remove any debris that may be in the bladder, which can lead to blocking the catheter, preventing it from draining
  • PEG – bolus and continuous
    Our Professional Carers can assist with a PEG for medication,  including routine flushing, as well as a PEG for nutrition including routine flushing.
  • Nebuliser
    Our Professional Carers are able to support a client with suctioning of the mouth.
  • Insulin injections
    Our Professional Carers  are able to assist with Insulin administration using an insulin pen.
  • Oxygen therapy
    Our Professional Carers are able to assist you with the use of cough assist machines.
  • Suppositories
    Our Professional Carers can manage convene catheters by emptying bags, applying and/or removing a convene, detaching and attaching night bags, cleaning the entry site and escalating any concerns or changes in urine output or quality.

 

  • Convene catheter management
    Our Professional Carers can liaise with the clients community nursing team, to ensure the client receives the suppositories they require for their care.
  • Diabetes blood sugar monitoring
    Our Professional Carers can risk assess a client’s situation to support blood sugar monitoring. This support is dependent on the level and stability of a client’s diabetes.
  • Inhalers
    Our Professional Carers can administer inhalers as prescribed to the client.
  • Support with syringe drivers
    Our Professional Carers look after clients who use syringe drivers, and while these are managed by the community nursing team, they can monitor the site and the surrounding skin and escalate any concerns.
  • Medications management
    Our Professional Carers can administer medication as per the prescribing instructions.
  • Colostomy support
    Our Professional Carers can provide support with a colostomy by monitoring the area, emptying the bag and ensuring the area is kept clean.
  • Continence care
    Our Professional Carers can support and help manage continence, they can manage catheters, convenes, pad use and assist with toileting/wiping.
