How we work with our clients
As a healthcare professional, you need to know how we implement our care when working with our clients and their families to ensure they receive highly personalised care that meets their needs - while ensuring you are kept abreast of progress. Here is how we work with clients:
1. Client contacts The Good Care Group
Our experienced care advisors listen to clients’ needs to fully understand their situation.
2. Care needs assessment
Our Regional Care Manager carries out a comprehensive and holistic care assessment, focussing on medical, physical, emotional and social needs, as well as domestic requirements. A proposal on the home care service is then provided to the client with the associated cost.
3. Care planning
Once a family has decided they would like our service, we produce a plan of care that is agreed with the client. Care planning is a responsive and evolving process and one that we manage throughout the client’s time with us.
4. Carer matching
The next step is to identify a dedicated care team of two professional carers who work opposite each other on the rota pattern that best meets the client’s needs and provides continuity of care. Clients are presented with carer profiles and the opportunity to meet carers ahead of starting our service.
5. Starting a home care service
We agree a start date and plan the service start with the client to ensure that everything goes smoothly from the outset. On the first day, an experienced manager accompanies the carer.
6. On-going support
We provide families with peace of mind; right from the very first time they contact us. Plans of care are reassessed frequently and our Regional Care Managers maintain regular contact and make visits as required.
Every month we provide feedback on a client’s progress to multidisciplinary healthcare and social care professionals involved in care for the client.
