Working to improve client health outcomes

We are committed to improving client outcomes through robust care planning and risk management, which include rigorous quality assurance processes. Almost 90% of our clients are living with chronic and progressive conditions and through delivering our holistic care, the majority of people show improved health and wellbeing outcomes.

Hospital admissions

10% of care home residents were admitted to hospital in 2013 with a potentially avoidable condition defined as a bone fracture, dehydration, pneumonia or respiratory tract infection.  Only 6% of our clients were admitted to hospital for these reasons. Our proactive approach to managing health and wellbeing has kept 75% of our clients out of hospital. For the 25% admitted our re-admission rate is below 5%. 

Antipsychotics and management of dementia

90% of all people living with dementia experience behavioural symptoms at some point, and in care homes around 20% of people are prescribed antipsychotic medication.  Improved health and behavioural outcomes are achieved through our person-centred and highly specialist approach to dementia care at home, which focuses on understanding and listening to the person. This compares to only 45% of our clients experiencing behavioural challenges despite many living with advanced dementia, and less than 10% of our clients are currently prescribed antipsychotic medication.

We work closely with GP’s and community mental health teams to put psycho-social care strategies in place which promote wellbeing and reduce behaviours that challenge without the need for medication. 

Urinary Tract Infections (UTI's) 

UTI’S are the most common non-respiratory infection in care homes with an incident rate of up to 50% as environments provide the opportunity for transmission of infection. These infections drive falls, behavioural challenges and deterioration in health, resulting in increased hospital admissions and pressure on primary care.

We put robust risk management plans in place to avoid urine infections, and ensure staff are trained to identify the symptoms early so that the correct medical treatment can be sought and crisis avoided. 65% of our clients are UTI free and only 10% of clients have had 2 or more UTI’s in last three months.
